Full Life Transitions serves individuals 25+ y/o who have successfully completed residential treatment and recognize that they can strengthen their recovery by participating in a sober living environment during the first 3-12 months of recovery. Individuals who are 22-24 y/o will be considered on a case-by-case basis.
Because we have well-established working relationships with several professional monitoring organizations, Full Life Transitions primarily serves impaired professionals but we welcome applications from anyone 25+y/o seeking to strengthen their recovery.
Our all-inclusive transitional program includes:
Supervised recovery residency with others in recovery
Premier apartment community living at The Stratford at Hillcrest in Winston-Salem, NC
Weekly individual therapy, relapse prevention group, and process group with licensed counseling professionals (LPC or LCAS)
Daily meditation group, weekly skills group, weekly community group, recovery crisis support, and medication oversight
Referral to local recovery-friendly psychiatry practice
Participation in at least 6 mutual support meetings per week, including in-house meeting on Sunday evenings, and working with a sponsor
15 or more hours per week engagement in purposeful/productive activity through paid employment, volunteerism or school
Food allowance and drug testing included in monthly fee
Up to $760/month can be submitted to insurance companies for reimbursement from out-of-network benefits
Minimum 90-day stay

Complete pre-screening appointment together with referring primary therapist or continuing care coordinator
Complete phone interview with existing residents in the program
Submit Full Life Transitions application, rental application and financial agreement at least 48 hours before enrollment
Complete orientation process on move-in day and begin full program participation from day one.
